Typical Band 6 roles

Band 6 covers specialist nurses, health visitors, experienced paramedics, biomedical scientists and many senior clinician or team leader posts. Two Band 6 jobs can still have completely different rotas, so basic salary alone is a poor comparison.

Basic pay in 2026/27

England Band 6 on the NHS Employers 2026/27 scales runs from £39,959 at entry to £48,117 at the top point. Scotland’s Band 6 points are published through the Scottish Government’s AfC pay pages and PCS(AFC)2026/01; on the figures we model, the Scottish basic sits higher before tax.

At these salaries, income tax, National Insurance and pension contributions are material. A promotion that looks like several thousand pounds more basic can shrink once PAYE, pension tier and student loan lines have moved.

Why net pay jumps around at Band 6

Pension tiers step up with pensionable pay. Cross a boundary and the higher percentage applies to relevant pensionable pay, not just the slice above the line. Unsocial enhancements under Section 2 and HCAS under Annex 9 can be enough to tip you over.

In Scotland, Scottish Income Tax rates on middle incomes are more visible at Band 6 than at Band 5. In London, Outer or Inner HCAS can outweigh a Scottish basic advantage depending on the exact points you compare.

Section 2 rates for Band 6 are time plus 30% on nights and Saturdays and time plus 60% on Sundays and public holidays — the Band 4–9 schedule in the national table.

Band 5 to Band 6 moves

Before you accept a promotion, model both sides with the same hours and loan plan. Add each job’s unsocial pattern separately if the rotas differ. Confirm whether you start at Band 6 entry or assimilate higher — that detail is often clearer in the offer paperwork than in the advert.
